Hallux Abducto Valgus (Bunion)



Definition

Hallux Abducto Valgus (HAV) is characterised by lateral deviation of the hallux on the first metatarsal. There is abduction & valgus rotation of the hallux, whilst the first metatarsal is in an adducted & varus position.

Inadequate footwear - Particularly narrow toed &/or high heeled shoes exacerbate & enhance the progression of the deformity, rather than be the initial instigating factor.

Symptoms
  • Pain on the medial prominence of the 1st metatarsal head
  • Synovial thickening & inflammation at the above site
  • Secondary osteoarthritis of the 1st metatarsophalangeal joint (MPJ) which leads to narrowing of the joint space and osteophytic changes
  • Restricted motion at the 1st MPJ
  • Pressure lesions and nail deformity due to deformity:
Podiatric Treatment
  • The aim of conservative treatment is to relieve pain & pressure & to slow down its physical progression
  • Orthotics to address excessive pronation and ultimately offload the MPJ
  • Debridement of callus & corns.
  • Protective padding over the prominent joint
  • Extra width & extra depth footwear to accommodate the deformity
  • Anti-inflammatory or analgesic medications may be required for temporary pain relief or to reduce inflamed joint / bursa
  • Icing
  • Surgery - Often considered when bunion pain is interfering with daily activities
General Foot Care - Quick Tips
  • Wash your feet daily. Rinse off all soap and dry thoroughly, especially between toes.
  • Trim nails straight across, and not too short. Don't cut out or dig at corners.
  • Do not trim, shave, or use over-the-counter medicines to dissolve corns or calluses
  • Wear clean socks or stockings, changed daily. Don't wear any that are too short or too tight.
  • Wear shoes that fit.
